Use this form to request computer hardware or software purchases.
- Only CU Medicine Directors and Managers are authorized to request computer hardware or software.
- If the request is for a PC that will be located at a University of Colorado Anschutz Campus or UCH facility, please provide the Department Manager’s name.
- A separate form is required for each user who will be using the computer hardware or software. Please enter the user’s last name, first name, location, and jack number on each form.
- If you are ordering a department printer or other equipment not specific to a single user, then enter the name of the department.

Information Services will complete this request within 3 business days of date received. When the request has been completed, it will be sent back to the Director or Manager. CU Medicine Finance will require that an approved form be attached to any paperwork requesting computer hardware or software.
